Waist-to-height ratio (WHtR) is one of the simplest health screens — keep your waist under half your height. Multiple meta-analyses show WHtR predicts cardiometabolic risk (diabetes, heart disease, all-cause mortality) better than BMI because it directly captures abdominal fat, the metabolically active tissue that drives most weight-related disease.
The 'keep your waist under half your height' rule
The threshold of 0.5 (waist = half of height) was popularized by Dr. Margaret Ashwell and validated across more than 30 studies. It works the same in men and women, across most ethnicities, and from age 5 through old age — a rare property for any single health metric. A 5'10" (70 in) adult should keep their waist under 35 in. A 5'4" (64 in) adult should aim for under 32 in.
Above 0.5 indicates increased cardiometabolic risk. Above 0.6 is consistently associated with substantially elevated risk of type 2 diabetes, hypertension, and cardiovascular disease in long-term cohort studies. Below 0.4 may indicate underweight or insufficient lean mass and is also worth a conversation with a clinician.
Why WHtR beats BMI for individual screening
BMI ignores fat distribution. Two people with the same BMI of 27 — one carrying weight in the hips and thighs, the other in the abdomen — have very different metabolic risk profiles. The abdominal-fat person is at significantly higher risk for insulin resistance, fatty liver, and cardiovascular events even at the same total weight.
WHtR captures this directly. A 2012 meta-analysis in Obesity Reviews (300,000+ adults) showed WHtR outperformed BMI as a predictor of diabetes and cardiovascular disease. A 2020 meta-analysis in Nutrition Reviews extended this to all-cause mortality. The takeaway: if you only track one body metric, waist-to-height beats BMI.
How to measure waist correctly
Use a soft tape, stand relaxed, and exhale normally. Measure at the midpoint between the lowest rib and the top of the iliac crest (hip bone) — for most adults this is at or just above the navel. Don't suck in or hold the tape too tight.
Measure at the same time of day for tracking — first thing in the morning, after using the bathroom, before eating or drinking, gives the most consistent reading. Round your height to the nearest quarter-inch and use it consistently. A 1-inch drop in waist with no change in height moves your ratio meaningfully and usually corresponds to 4–6 lb of fat loss.
Three worked examples
A 5'10" man (70 in) with a 34 in waist has a ratio of 34 ÷ 70 = 0.486. Half his height is 35 in, so he is an inch under the threshold and sits in the healthy band. His BMI at 180 lb would be 25.8 — technically 'overweight' — which is a good illustration of why the two metrics disagree and why waist is the one clinicians increasingly trust.
A 5'4" woman (64 in) with a 36 in waist has a ratio of 36 ÷ 64 = 0.563. Half her height is 32 in, putting her 4 in over. That lands in the 'overweight, consider action' band even if her weight looks unremarkable on a scale — the ratio is picking up abdominal fat distribution that a BMI of 27 would not distinguish from muscle or hip-carried weight.
A 6'2" man (74 in) with a 47 in waist has a ratio of 47 ÷ 74 = 0.635, crossing into the highest-risk band. Getting under 0.5 means a 37 in waist — a 10 in reduction, which at roughly 4–6 lb of fat loss per inch implies 40–60 lb. That is a two-year project at a sustainable pace, and the useful framing is that the first two inches deliver a disproportionate share of the metabolic benefit.
In metric the arithmetic is identical because the ratio is dimensionless: an 86 cm waist on a 178 cm frame is 86 ÷ 178 = 0.483, the same healthy reading as the first example. You never need to convert units, only to use the same unit for both measurements.
What to do if you are above 0.5
Visceral fat is unusually responsive to modest, sustained changes — it is typically the first fat mobilized in a calorie deficit, which is why waist measurements often improve faster than scale weight. A 5–10% reduction in body weight commonly produces a disproportionate drop in waist circumference, and clinical trials have shown meaningful improvements in blood pressure, triglycerides, and insulin sensitivity at that level, well before reaching any 'ideal' weight.
The interventions with the strongest evidence are unglamorous: a moderate calorie deficit (roughly 500 kcal/day), protein at 0.7–1.0 g per pound of goal weight, resistance training two to four times a week to preserve lean mass, and 7,000–10,000 daily steps. Reducing liquid calories and alcohol tends to move abdominal fat specifically, since both are handled by the liver and contribute to hepatic fat accumulation.
Track the ratio monthly rather than weekly. Waist measurement has roughly ±1 cm of day-to-day noise from bloating, hydration, and tape placement, so a weekly reading mostly measures noise. A monthly cadence over six months shows a real trend.

Limits — who this screen does not fit
Waist-to-height ratio is a screening tool, not a diagnosis, and there are populations where it misreads. It is not valid during pregnancy, where abdominal circumference reflects the uterus rather than adipose tissue. It is unreliable in people with ascites, large abdominal hernias, or significant bloating from gastrointestinal conditions.
Very muscular individuals with thick oblique and abdominal development can measure above 0.5 without excess fat, though this is far rarer than the gym-floor claim suggests — a genuinely lean athlete usually still measures under 0.5 because the muscle adds little circumference relative to height. If you can see abdominal definition and measure over 0.5, a body-fat estimate is the better next step.
Ethnicity shifts risk thresholds. Populations of South Asian, Chinese, and Southeast Asian descent develop cardiometabolic complications at lower waist circumferences, and some guidelines apply a stricter cutoff nearer 0.48. Conversely, a single global threshold slightly overcalls risk in some Pacific Islander populations. The 0.5 rule remains the best single number across groups, but it is a starting point for a conversation, not a verdict.
For children the 'waist under half your height' rule applies from about age 5, and it is one of the few body metrics that needs no age- or sex-specific percentile chart. Below age 5, use growth charts and defer to a pediatrician. At any age, a ratio above 0.6 or a rapid change over a few months warrants a clinical assessment rather than a self-directed plan.
